Scoring

Total possible: 36 points

  • 30–36: Healthy. Pipeline is a reliable planning asset. Maintain the discipline that got you here.
  • 21–29: Functional but fragile. Specific weak spots exist — identify your 3 lowest-scoring questions and treat those as this month's priority.
  • 12–20: At risk. Multiple structural issues are compounding. The forecast number cannot be trusted without manual scrutiny of every deal.
  • Below 12: Unhealthy. Pipeline math will not predict revenue. Stop and rebuild qualification and stage discipline before trusting any rollup number.

The single most important question this template forces

After scoring, ask: which single question, if fixed first, would move the most other scores with it? Usually it is Q1 (confirmed next steps) or Q10 (shared stage definitions) — both are upstream of most of the others.

How to use it

Pull your CRM data and answer all 12 questions honestly before a QBR or pipeline review, total your score for the health band, then take your three lowest-scoring questions straight into the matching companion resource.
